All Family Home Care
All Family Home Care serves as a resource for our patients, the patient’s family, and the community. We provide health care education for the patient, the patient’s family and for the community.
As an employer, All Family Home Care practices non-discrimination and strives to provide opportunities for personal and professional growth. We implement methods of screening applicants to hire only the best qualified into our staffing and agent pool, regularly evaluating their progress in order to be able to provide quality personnel. We provide staff development opportunities an∂ programs,
including orientation and programs concerning current practices in nursing and allied health care. The counties that we serve are Ramsey, Hennepin, Washington, Anoka, Wright, Carver, Scott and Dakota
Our hours of operation are 8:00 am to 5:00 pm, Monday through Friday

All Family Home Care will directly provide skilled nursing services provided by Registered Nurses and Licensed Practical/Vocational Nurses and para- professional services provided by home health aides. We will also provide Physical, Occupational and Speech Therapists, Medical Social Workers hired by us or contracted for by us to provide care to patients transitioning from hospital to home, and improve independence, ability to manage at home and reduce the likelihood of returning to the hospital.
We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the patient and professional’s point of view.
